Factor health buckets the agent’s signals by trust factor and reports the success rate for each, plus how many pieces of evidence backed it and whether it meets the minimum its tier requires. Low evidence is shown honestly as "no data" rather than assumed healthy — absence of evidence is not evidence of health.
